Update the trusted Forgejo review writer allowlist to match the active roster: one smart slot plus three Codex Luna slots for both approach and code lanes.

Retires the old approach-smart-2, approach-3, and code-smart-2 slot names from the post-review action.

Validation: format, typecheck, lint, knip, and 56 tests pass. FTA could not run because the repository does not declare the fta-cli peer binary.
